Many departments request ID cards and electronic services for individuals who are not regular students, faculty, or staff by filling out the Application Form for IDs below:

Requisites for letter submissions:

A. From Students

  1. Date
  2. Complete name of the addressee
    (Rev. Fr. Louie R. Coronel, O.P., EHL)
  3. Salutation
  4. Body of the letter
  5. Printed Name and Signature of Student, including student no. and name of the program
  6. Printed Name and Signature of Adviser or Program Chair (Noted by)
  7. Printed Name and Signature of Dean / Asst. Dean / Faculty Secretary (Endorsed by)

B. From Office of the Dean/Director

  1. Date
  2. Complete name of the addressee
    (Rev. Fr. Louie R. Coronel, O.P., EHL)
  3. Salutation
  4. Body of the letter
  5. Printed Name and Signature of Dean / Asst. Dean / Faculty Secretary (these are authorized signatories)

 C. From a Faculty

  1. Date
  2. Complete name of the addressee
    (Rev. Fr. Louie R. Coronel, O.P., EHL)

  5. Printed Name and Signature of the Faculty Member
  6. Printed Name and Signature of Program Chair (Noted by)

Requisites for Letter of request for late enrollment

  • Signed letter by the student, noted by the Program Chair,
    Endorsed by the Dean
  • Attachment must include attendance duly prepared by the course facilitator
  • An official receipt or bank receipt as proof of payment, if enrollment processed online.
