Note:
- Graduates may apply for transcript of records in person or through an authorized representative via the “express lane”, which makes the transcript available in twenty-four (24) hours.
- Students/graduates who have already been issued their Transfer Credentials (Honorable Dismissal) may not apply for records online. They may apply in person provided they submit a letter of no-objection from the Office of the Registrar of the school which admitted them.
- Transfer Credential (Honorable Dismissal) is issued relative to the program last enrolled in. If the applicant is not a graduate, he should apply in person or through an authorized representative.
- Request will be processed only upon confirmation of the University Cashier that the remittance has been received.
- Authentication of records should be applied for in person or through an authorized representative.
If the records will not be claimed personally or through a representative, the same may be forwarded by mail or by courier. Corresponding payment has to be made and forwarding address provided.
