Application

Application Requirements

A. FOR FILIPINO GRADUATES OF LOCAL SCHOOLS

  1. National Statistics Office (NSO) certified true copy of birth certificate (photocopy is acceptable provided a true copy is shown for verification)
  2. Certificate of naturalization for naturalized Filipinos.
  3. Two (2) signed most recent colored 2x2 photographs.
  4. Certified true copy of transcript of records ( 3 1/2  years for those graduating in March.
  5. Photocopy of diploma/s of parent/s if UST graduate/s.
  6. Photocopy of NMAT score.
  7. Three (3) Certificates of Good Moral Character (one each from the Academic Dean, Office of Students Affairs   and a Professor). 
  8. Properly accomplished application form.
  9. Processing fee of One Thousand five hundred  pesos (Php 1,500.00) 

FOR FILIPINO GRADUATES OF FOREIGN SCHOOLS

  1. Same as for A except items A4 and A9

 

FOR RESIDENT ALIENS

     All of the above except A2 plus the following:

     1. Photocopy of ICR (Immigrant Certificate of Registration)

     2. Notarized affidavit of support

     3. Processing fee of three hundred US dollars ($ 300.00) 

 

 

 

FOR  NON- RESIDENT ALIENS

  1. Certified true copy of birth of birth certificate (notarized copy is acceptable).
  2. Certificate of citizenship.
  3. Two (2) signed most recent colored 2x2 photographs.
  4. * Transcript of records of a completed 4-year BSor BA course
  5. Photocopy of diploma/s of parent/s if UST graduate/s
  6. Photocopy of NMAT score.
  7. Three (3) certificates of good moral character (one each from Academic Dean, Office for Student Affairs, and a Professor)
  8. *Notarized affidavit of support with bank statement
  9. Two (2) original copies of Personal History forms
  10. Police Clearance from place of residence
  11. Properly accomplished application form
  12. Processing fee of three hundred US dollars ($300.00)

*Transcript of records, notarized affidavit of support, and police clearance must be authenticated by the Philippine Consulate Office or Embassy - must bear the red ribbon of the Consulate.

 

APPLICATIONS FROM THE FOLLOWING WILL NOT BE CONSIDERED

     1. Those who are not holders of a Bachelor's degree in Science or the Arts (BS/BA)   conferred by a duly recognized institution at the time of application for non-resident aliens and for Filipinos studying in foreign schools

     2. Those who have incurred more than eight (8) units of failure  

     3. Those who have advanced credits in Medicine

     4. Those who have been convicted of crime.
